Lets start with SCOM 2012 R2 installation.
I have already configured SQL 2012 SP1 on Windows Server 2012. All other servers other than SQL server is Windows 2012 R2. For SQL, you need to have Analysis Services installed. Also start the SQL agent.
PART1: SCOM 2012 R2 Installation
The wizards are very clear and it also checks and shows you all prerequisites. I will put all roles on one VM, namely, Management Server, Operations Console, Web Console, Reporting Server:

You need to have SQL Server Reporting services installed on the same machine. Once installed have it configured with the DB (residing on the DB server) and configure its URLs. Then you will be presented with the local SSRS instance in the Wizard:
